Complete, save and return (email or drop off) the fillable PDF forms, and attach identifications (Birth Certificate or Passport) to Lori at cabri_school@chinooksd.ca

If you live in a rural area or require transportation to Evergreen Early Learning you will also have to fill out the Transportation Request Form
